2009 OASPA/OCPA Annual Conference

Registration Instructions:
* If registration form or payment will not be recieved until after the January 9, 2009 deadline please be sure to include the $25.00 late fee*
- Open the registration form here.
- Fill out participant information. Each participant will need his/her own registration form.
- Participants should be members of OCPA or OASPA. If you need to register someone as an OCPA member please do so from the membership page and pay for it separately.
- Select form of payment. (PayPal, institutional or personal check)
- PayPal users will proceed through our payments page.
- You will be asked to include your PayPal confirmation # or check # if applicable on each registration form.
- Select to print (to later fax/mail) or email form.
- If emailing form, it is recommended that you have Adobe Reader 9.0. It is available as a free download at www.adobe.com You may automatically email the single form data through a desktop based email system or save the data to your computer to send later as a registration group or through an internet based email system. Files will be in data (.xml) format.
- Participants will recieve a confirmation email and directions to the conference from the conference committee.
